Health Forms

If your child is participating in any of our weekly classes (Turtle Tots or After school Clubs) or camps, please be sure to have a health form completed and given to us by the first class. This ensures the health and safety of all of our participants!

You can download the health form here:  Camper_Health_and_Emergency_Form.pdf

Parent/Guardian Handbook

Please read through our Parent/Guardian handbook for useful information about Arboretum program rules and procedures. All program participants need to sign and return the agreement (found on the last page) two weeks before the start of a camp and/or Arboretum program. You can download the Parent/Guardian Handbook here: Parent Handbook 2016
